If you would like to make a gift of stock to the Crossroads Fund, contact your bank or broker and provide them with the following account information:

Also, on or before the day you contact your broker to transfer the stock, complete the form below. Without this information, there may be a delay in the receipting process. Submitting this form to Crossroads will allow us to follow up with the broker to make sure the stock is immediately sold upon transfer. This form is also available as a printable PDF.
